2. Inspect the evidence
Review the relevant code, logs, infrastructure, configuration, and recent changes.
3. Diagnose the failure
Trace the cause so the next change has a clear reason behind it.
4. Repair or plan
Repair the issue when practical or provide a clear remediation plan. Immediate resolution is not guaranteed; availability and scope are confirmed first.
